Home Health LPN Job Details

Licensed Practical Nurse Job Requirements
- Current Licensed Practical Nurse (LPN) license
- At least 1 year of recent home health LPN experience
- Proficiency with HCHB is required
- Ability to work independently and travel throughout the assigned South Puget Sound service territory
- Strong clinical judgment, communication, organization, and healthcare documentation skills
Home Health LPN Responsibilities
- Provide skilled nursing care to patients in their homes across assigned counties
- Assess patient conditions, monitor progress, and communicate changes to the interdisciplinary care team
- Administer medications and treatments in accordance with each patient’s plan of care
- Educate patients and caregivers on disease management, medications, safety, and available healthcare resources
- Complete accurate and timely clinical documentation in HCHB
- Meet a minimum productivity expectation of 30 points per week
- Complete required documentation by 10:00 a.m. on the following day
- Collaborate with care team members to support quality outcomes and continuity of care
