Home Health Physical Therapist Job Details

• Weekly Estimated Pay: $2,310-$2,480
• Location: Chesapeake, Virginia, in the Coastal Virginia region
• Healthcare Setting: Home Health
• Start Date: October 19, 2026
• Travel Contract Duration: 13 weeks, through January 18, 2027
• Schedule: Monday-Friday, 8:00 a.m.-5:00 p.m.
• Shifts: 5 shifts per week
• Shift Duration: 8-hour days
• Hours: Approximately 40 hours per week
• Caseload: Approximately 6-7 weighted visits per day
• Weekend Coverage: Approximately every fourth or fifth weekend for PT cases; occasional weekend coverage may be needed for orthopedic cases
• Travel Experience: First-time travel physical therapists are welcome

Physical Therapist Job Requirements

• At least 1 year of professional physical therapy experience required
• Previous Home Health Physical Therapist experience preferred
• Active Virginia Physical Therapist license or compact privilege required at submission
• Current CPR certification required
• Experience with MatrixCare or Epic preferred, but not required
• Reliable transportation for community-based patient visits
• Ability to complete required pre-start orientation and compliance modules
• Successful completion of required background, license, and fit-testing requirements

Home Health Physical Therapist Responsibilities

• Evaluate patients in their homes and develop individualized, medically prescribed physical therapy plans of care
• Administer therapeutic treatments for patients recovering from injuries or managing muscle, nerve, joint, and bone conditions
• Provide patient and caregiver education to promote safety, mobility, and continued progress between visits
• Direct patients through selected activities to restore, reinforce, and enhance physical performance
• Monitor patient response to treatment and adjust interventions as clinically appropriate
• Participate in discharge planning and coordinate care with the interdisciplinary healthcare team
• Complete accurate, timely clinical documentation in accordance with applicable policies and professional standards
• Manage a daily community-based schedule while delivering compassionate, patient-centered rehabilitation care
