Compassionate Home Health RN needed for a 13-week assignment in Forty Fort, PA. Join a homecare team providing skilled nursing visits, start of care, recertifications, resumptions of care, and discharges across Luzerne County.
We are seeking a Registered Nurse for a 13-week homecare assignment in Forty Fort, Pennsylvania. This on-site home health nursing opportunity is ideal for an RN interested in managing a dedicated territory and caring for patients in the home setting. Located in the scenic Wyoming Valley near the Susquehanna River, the Forty Fort area offers a welcoming community atmosphere with convenient access to outdoor recreation, local dining, and the greater Wilkes-Barre region.
Job Details
- Weekly Estimated Pay: $2290-$2660
- Location: Forty Fort, Pennsylvania
- Setting: Homecare / Home Health Nursing
- Start Date: 10/19/2026
- Duration: 13-week assignment
- Schedule: Monday through Friday, 8:00 AM – 4:30 PM
- Shift Duration: 8-hour days
- Shifts Per Week: 5
- Hours Per Week: 40
- On-Call/Weekend Requirement: Every 3rd weekend and 1 night of on-call per week
- Coverage Area: Luzerne County
- Mileage Reimbursement: Reimbursed between patient visits
Job Requirements
- Active Registered Nurse (RN) license
- Recent home health or homecare experience is helpful
- Experience with HCHB is a plus
Responsibilities
- Manage a designated patient territory as the RN case manager
- Perform skilled routine home visits for assigned patients
- Complete start of care, resumption of care, recertification, and discharge visits
- Provide cross-coverage for team members within assigned territories as needed
- Provide 100% patient care in the home setting
