Job Requirements
- Minimum of 12 months of home health occupational therapy experience within the past 3 years
- Active Occupational Therapy license or eligibility in Wisconsin
- Experience with Homecare Homebase (HCHB) preferred
- Strong clinical judgment, communication skills, and independent time management
- Ability to travel within the assigned territory as needed
Responsibilities
- Provide skilled occupational therapy services to patients in their homes
- Evaluate patient needs and develop individualized treatment plans
- Support patients in improving mobility, activities of daily living, and functional independence
- Document visits, treatment plans, and progress accurately and promptly
- Collaborate with the interdisciplinary care team to support quality patient outcomes
- Deliver compassionate care while maintaining a safe and efficient workflow
