- Assignment Length: 14 weeks
- Start Date: 08/24/2026
- Schedule: Monday through Friday, 8:00 AM to 4:30 PM
- Shift Duration: 8-hour shifts
- Hours per Week: 40
- Weekly Estimated Pay: $2,030 – $2,190
- On Call / Weekends: No on-call and no weekend coverage
- Mileage: Reimbursement available between patient visits
- Productivity Expectation: 30 points
Job Requirements
- At least 1 year of Physical Therapist experience preferred
- Master’s degree required
- Active Physical Therapist licensure or eligibility to practice in Pennsylvania
- Home health therapy experience strongly preferred
- OASIS experience preferred
- Professional certification required
- Strong communication, organization, and time-management skills
Responsibilities
- Provide skilled physical therapy services in the home health setting
- Evaluate patients and develop individualized treatment plans
- Implement therapeutic interventions to improve strength, mobility, balance, and functional independence
- Document care accurately and maintain timely clinical records
- Collaborate with the care team to support safe, effective patient outcomes
- Travel within assigned coverage areas to deliver patient care
- Educate patients and caregivers on home exercise programs and safe mobility strategies
