- Schedule: Monday through Friday, 8-hour day shifts, approximately 32 hours per week
- Estimated Weekly Pay: $2,030 – $2,190
- On-Call/Weekend Requirement: None
- Coverage Areas: Christian, Trigg, Caldwell, and Lyon counties
- Patient Care Focus: Home-based rehabilitation, mobility support, and functional recovery
Job Requirements
- Active Physical Therapist license or eligibility to practice in Kentucky
- Doctoral degree in Physical Therapy required
- Home health experience preferred
- Strong clinical skills in evaluating mobility, function, and safety in the home setting
- Experience with HomeCare HomeBase preferred
- Excellent communication, time management, and patient education skills
- Professional, dependable, and comfortable working independently in the field
Responsibilities
- Evaluate patients in the home setting and develop individualized plans of care
- Support patients in improving physical function, mobility, and safety at home
- Provide skilled therapy interventions aligned with each patient’s goals and progress
- Educate patients and caregivers on exercises, safety strategies, and functional mobility techniques
- Document care accurately and timely according to home health standards
- Coordinate with interdisciplinary team members to support optimal patient outcomes
- Manage a productive schedule while maintaining high-quality, patient-centered care
