Home Health Physical Therapist Job Details

• Healthcare Setting: Home health
• Location: Corpus Christi, Texas, and surrounding communities
• Start Date: October 26, 2026
• Assignment Duration: 13 weeks, ending January 26, 2027
• Weekly Estimated Pay: $2,030-$2,190
• Schedule: Daytime coverage with 8-hour shifts
• Hours: Approximately 40 hours per week, with a 32-hour weekly guarantee
• Weekend Requirement: One weekend every six weeks preferred
• Coverage Area: Corpus Christi, Portland, Taft, Ingleside, Aransas Pass, Rockport, Sinton, Odem, Beeville, Mathis, and Robstown
• Dress Code: Scrubs of any color

Physical Therapist Job Requirements

• At least 1 year of professional Physical Therapist experience; home health experience required
• Doctor of Physical Therapy degree or equivalent qualifying education
• Current, unrestricted Physical Therapist license in Texas
• Strong clinical assessment, documentation, and patient education skills
• Ability to travel independently between patient visits throughout the assigned service area
• Dependable, organized, adaptable, and committed to patient-centered home health care
• Proficiency with OASIS documentation preferred
• HCHB experience preferred

Home Health Physical Therapist Responsibilities

• Perform start-of-care visits, comprehensive physical therapy evaluations, reassessments, and discharge visits in the home health setting
• Develop individualized physical therapy plans of care based on patient needs, functional goals, and clinical findings
• Provide therapeutic exercise, mobility training, balance activities, gait training, and other appropriate physical therapy interventions
• Educate patients and caregivers on home exercise programs, safe mobility, fall prevention, and the use of assistive devices
• Monitor patient progress and modify treatment plans to support measurable functional improvement
• Complete timely, accurate clinical documentation, including OASIS-related requirements when applicable
• Communicate effectively with patients, families, and members of the interdisciplinary care team
• Coordinate care across a broad geographic service area while maintaining dependable visit coverage
