Home Health Registered Nurse (RN) Case Manager – Ohio Contract Job

Home Health RN Job Details
- Location: Uniontown, Ohio, serving patients throughout Summit, Portage, and Medina counties
- Weekly Estimated Pay: $2,120-$2,230
- Start Date: October 12, 2026
- Contract End Date: January 8, 2027
- Contract Duration: Approximately 13 weeks
- Schedule: 8-hour shifts, approximately 8:00 a.m.-4:30 p.m.; 40 hours per week with 36 hours guaranteed
- On-Call and Weekend Requirement: One on-call week and one weekend during the assignment
- Travel: Mileage reimbursement between patient visits, from the first patient to the last patient
- Patient Care Setting: Home health
- Dress Code: Navy or black scrubs
Home Health Registered Nurse Job Requirements
- Current Registered Nurse (RN) license in the state of Ohio
- At least 1 year of professional Registered Nurse experience
- Associate degree in Nursing or equivalent educational preparation
- Required professional nursing certifications and credentials in good standing
- Strong patient assessment, clinical judgment, documentation, and care coordination skills
- Valid driver’s license and reliable transportation for community-based home health visits
- Previous home health nursing or case management experience preferred
- Experience with Homecare Homebase (HCHB) is a plus
Home Health RN Case Manager Responsibilities
- Serve as the case manager for an assigned home health patient caseload
- Perform comprehensive nursing assessments and develop individualized plans of care
- Provide skilled nursing interventions and monitor patient progress in the home setting
- Coordinate services with physicians, therapists, aides, caregivers, and other members of the interdisciplinary care team
- Educate patients and families about medications, disease management, treatment plans, and home safety
- Identify changes in patient condition and communicate clinical concerns promptly
- Complete accurate and timely clinical documentation in accordance with home health standards
- Support continuity of care through follow-up visits, reassessments, and interdisciplinary collaboration
- Provide on-call and weekend coverage as scheduled
