Travel Physical Therapist – Home Health | Bourne, MA

Overview
This is a full-time travel Physical Therapist position supporting home health services in a community-focused setting. You’ll work closely with patients and the care team to deliver high-quality rehabilitation therapy, complete evaluations, and guide patients through meaningful progress in their recovery journey.

- Weekly Estimated Pay: $2,030 – $2,190

- Mileage Reimbursement: Yes, between patient visits
- Productivity Expectation: 30 points
Job Requirements
- Bachelor’s degree required
- Active Massachusetts Physical Therapist license
- At least 1 year of Physical Therapist experience required
- Home health experience strongly preferred
- Professional certification required
- HCHB familiarity is a plus
- Strong communication, organization, and time-management skills
- Ability to work independently while supporting collaborative patient care
Responsibilities
- Perform start-of-care assessments, evaluations, and skilled therapy visits
- Develop and implement patient-centered treatment plans
- Provide education and support to help patients improve function and mobility
- Coordinate care with the team and provide handoff/supervision to PTA as appropriate
- Document visits accurately and in a timely manner
- Monitor progress and adjust treatment plans based on patient needs
- Maintain productivity goals while delivering high-quality care
- Support safe, effective rehabilitation in the home setting
