• Location: Charlottesville, NC (serving Mecklenburg and Union Counties)

• Employment Type: Travel Nursing Contract

Registered Nurse Qualifications

• Active Registered Nurse (RN) license in North Carolina or Compact State
• Minimum 1 year of clinical nursing experience
• Valid driver’s license and reliable transportation for homecare visits
• Preferred: Experience with HCHB, Wound Care, IV therapy, Ports, Wound Vac, and Oasis documentation
• Strong assessment, care planning, and communication skills

RN Case Manager Responsibilities

• Assess, plan, implement, and evaluate nursing care for patients in home and community healthcare settings
• Deliver case management, health maintenance, and disease prevention services
• Supervise care plans and support clinical staff
• Coordinate with physicians and healthcare professionals to ensure comprehensive patient care
• Oversee medication administration, maintain accurate clinical documentation, and ensure compliance with healthcare policies
• Monitor and report changes in patient conditions to physicians and care teams
• Complete all required nursing documentation (daily, monthly, quarterly, annual)
• Maintain infection control standards for medical equipment and supplies
• Schedule and follow up on patient health-related appointments
• Provide training for new employees and annual recertification on health and safety procedures
• Ensure unlicensed staff are trained on patient-specific health needs
