- Contract Duration: 13 weeks
- Schedule: Day shift, 8-hour shifts (8:00 AM – 4:00 PM)
- Shifts Per Week: 5
- Hours Per Week: 40
- Orientation: 16 hours
- Charting: Homecare Homebase; Cerner experience helpful
Job Requirements
- Active Tennessee PTA license or compact license in hand
- At least 6 months of recent or current home health experience required
- Home health travel experience required
- Comfortable working independently in a home care setting
- Experience with Homecare Homebase preferred
- Strong communication, organization, and patient care skills
- Ability to manage a home health caseload and document accurately
Responsibilities
- Provide skilled physical therapist assistant services to patients in their homes
- Support treatment plans developed by the supervising physical therapist
- Deliver patient-focused care while promoting mobility, safety, and independence
- Document home health visits and patient progress accurately in the charting system
- Collaborate with the interdisciplinary care team to help patients reach functional goals
- Manage daily visits efficiently while maintaining high-quality clinical care
- Adapt to a dynamic home health environment and occasional weekend needs as scheduled
