What You’ll Do
As a Home Health Physical Therapist, you will:

• Evaluate patients in their homes and assess physical abilities and functional limitations.
• Develop individualized treatment plans based on each patient’s goals and needs.
• Provide skilled physical therapy to restore, maintain, or improve physical function.
• Help patients manage pain and prevent further disability or functional decline.
• Educate patients and families on exercises, mobility, safety, and strategies for continued progress.
• Collaborate with interdisciplinary care teams to provide a coordinated patient experience.
• Monitor patient progress and adjust treatment plans as appropriate.
• Help patients achieve greater independence and quality of life.

Ideal Background
We are especially interested in Physical Therapists with experience in:

• Home health
• Geriatrics
• Long-term care / skilled nursing
• Other settings involving medically complex or older adult populations

Candidates coming exclusively from a traditional outpatient background may not be the best fit for this particular opportunity.
