Job Requirements
- Active LPN/LVN license required
- Minimum 1 year of nursing experience preferred
- Experience in long-term care, rehabilitation nursing, skilled nursing, or similar patient care settings is a plus
- Strong communication, teamwork, and patient-centered care skills
- Ability to provide compassionate support to patients throughout the recovery process
Responsibilities
- Provide direct nursing care to patients in a rehabilitation and long-term care setting
- Support individualized care plans for patients recovering from complex injuries and illnesses
- Monitor patient status and report changes to the care team
- Assist with daily patient needs, medication administration, and clinical documentation as assigned
- Collaborate with interdisciplinary team members to promote positive patient outcomes
- Deliver respectful, high-quality care that supports recovery, function, and quality of life
