Physical Therapist Job Requirements
- Bachelor’s degree required
- Current Physical Therapist license/certification
- At least 1 year of Physical Therapy experience preferred/required
- Prior home health experience strongly preferred
- Strong communication, organization, and time management skills
- Proficiency with HCHB preferred
- Ability to provide patient-centered care in a home health environment
Physical Therapy Responsibilities
- Assess and evaluate patient care needs related to mobility, functional status, endurance, stamina, pain, and other medical conditions
- Develop individualized treatment plans based on patient needs and clinical findings
- Perform therapeutic interventions that support improved mobility, independence, and overall well-being
- Monitor patient progress and adjust care plans as needed
- Collaborate effectively with the interdisciplinary care team to support positive patient outcomes
- Document visits and care plans accurately within the electronic medical record system
- Provide compassionate, high-quality care to patients in the home setting
